We are extremely pleased to inform you that we at Birmingham East office have starting to use an electronic care planning and care notes system.

Pass System - 

We are extremely pleased to inform you that we at Birmingham East office have starting to use an electronic care planning and care notes system. Our team members will still be visiting and providing the same high level of care & support to you. However, they will now be using a mobile based system to receive the care plan. They will then enter their care notes and medication records, on the mobile phone device to allow ‘live data’ which will continually improve our service.

Benefits
 

The system reduces by 50% the time and cost of setting up care plans for patients going home from hospital. The care plan can be created at the hospital and delivered direct to the care worker. The results of care (care notes, tracking of tasks being completed, MAR sheets, and outcomes being achieved) can be reviewed remotely to monitor care delivery and to assess the appropriateness of the care plan. The system has an enquiry and referral function that would support the creation of a minimum data set by social care workers in hospitals to then be shared with homecare providers. The system would support the assessment of social care needs in hospitals by social care workers using tablets on the wards. Bluebird Care Edinburgh and Glasgow, has reported 122 hours saved per month.
